Electing Vision Care Coverage
VSP - Vision Care
You may elect vision care coverage, which provides affordable, quality vision care nationwide. Although vision care services and supplies are covered in network and out of network, your benefits are generally greater when you use in network providers. Your cost are based on the family member you choose to cover.
Once enrolled, no ID card is necessary, and there are no claim forms to complete when you see a contracted provider.
If covered services are received from a non-participating provider, you are responsible for paying the provider in full. You or the provider must submit the itemized bill and a copy of your prescription with the Claim Form to VSP.
